I, Constantine Sigelakis, aged 52 years, occupation storekeeper, do declare on oath that my persona description is: color white, complexion fair, height 6', weight 195 pounds, color of hair gray/black, color of eyes blue, no visible marks. I was born in Sparta Greece on the 23rd day of April 1873; I now reside at 1786 Sheepshead Bay Road, Brooklyn, New York. I emigrated to the US from Pireaus, Greece on the vessel Algeria; my last foreign residence was Sparta, Greece; I am married; the name of my wife is Anna; she was born at Sparta, Greece and now resides with me...renounce allegiance to the present Government of Greece. I arrived at the Port of NY on or about the 16th day of September 1907.

4. I declared my intention to become a citizen of the US on July 27, 1925 i the US District Court of Eastern District of NY, at Brooklyn NY.
5. I am married. The name of my wife or husband is Anna; we were married on January 1907 at Sparta, Greece; she was born at Sparta, Greece on 1875; entered the US at New York on September 16, 1907 for permanent residence therein, and now resides at 1786 Sheepshead Bay Road, Brooklyn, NY. I have 9 children, and the name, date and place of birth...are:
