I have the honor to apply for my discharge to enable me to be mustered in the United States Service as 2nd Lieut. Co" "E" 45th Reg, Vet Vols. I was enrolled as 1st Sergt Co "E" 45th PA Vols Sept 2nd 1861 - mustered in United States as an enlisted man Sept 12 1861. by Capt' D. H. Hastings. I reenlisted Jan - 1st 1864 as Vet Vol and was mustered in the United States. Service Jan 1st 1864. I received the accepted a commission as 2nd Lieut Co. "E" 45th PA Vet Vol June 6th 1864- Said commission to date May 1st 1864. I have been doing duty as 2- Lt. Co "E" 45th PA Vet Vols since that time. There is a vacancy of 2" Lt. Co "E" 45th PA vet Vols. caused by discharge of 2" Pa John Irvin discharged per expiration of term of Service Jan 18 1865 I was taken prisoner Sept. 30 1864, was paroled February 21 1865 and eas exchanged March 20" 1865. I reported to my Regiment for duty April 23" 1865 I would respectfully request that I be mustered back to the [--] vacancy occa---ned by the discharge of 2nd Lieut John Irvin.
