4. I emigrated to the United States from Port Said Egypt on or about the 31st day of December 1904 and arrived in the U.S. at the port of Philadelphia on the 4th day of February 1905 on the vessel Revel.
5. I declared my intention to become a citizen of the US on the 8th day of September 1914 at New York, NY, in the Supreme Court of 1 J D S of N.Y.
6. I am married. My wife's name is Katerina (Kosta); she was born on the 15th day of June 1893 at Sparta, Greece and now resides at 401 West 28th Street. I have 4 children and the name, date and place of birth and residence of each said children is as follows:

"Police Seeking Hit-Run Driver Who Killed Boy"
In last paragraph of article: "Gus Markos, 35, of 56 W. 28th Street, Manhattan, was killed last night when a motor truck in which he was riding got out of control and overturned on the Jericho Turnpike at Huntington, L.I. Anthony Theodoseau and Edward Teddle, both of the same Manhattan address, were injured and taken to Huntington Hospital. Theodoseau suffered a fractured skull and Teddle was cut and bruised. James Michel, also of the same address, alleged to have been driving the truck, was arrested on a charge of second degree manslaghter and released in $1,000 bail for a hearing next Monday."
