I was born in Sparta, Greece on the 9 day of March 1884; I now reside at 125 N 16th Street, Philadelphia, Pa. I emigrated to the US from Naples, Italy on the vessel Albert; my last foreign residence was Sparta, Greece. ...renounce allegiance to George I, King of the Greeks, of which I am now a subject; I arrived at the Port of New York on or about the 19 day of November 1902...

Second. My occupation is pool billiard parlor keeper
Third. I was born on the 9 day of March 1884 at Sparta, Greece.
Fourth. I emigrated to the US from Naples Italy on or about the 1 day of November 1902 and arrived in the US at the port of NY on the 19 day of November 1902 on the vessel "Albert"
Fifth. I declared y intention to become a citizen of the US on the 31 day of July 1912 at Philadelphia, PA in the US District Court of East. District of Pennsylvania.
Sixth. I am married. My wife's name is Helen; she was born on 14 day of May 1894 at Sparta Greece and now resides at 564 Atlantic Ave, Brooklyn, NY. I have 3 children:
Stella born 22 March 1915
Louis born 13 May 1916
Christ born 18 July 1917
all in Brooklyn, and reside at 564 Atlantic Avenue, Brooklyn.
Seventh. ...renounce allegiance to... Alexander King of the Hellenes.
Eighth. I am able to speak the English language.
