Physical examination: Well developed, well nourished, aged male. Mitral losing of heart. Systalic blood pressure 160, diatalic 100.
Psychological examination: Station and gait not obtained. Face just slightly exaggerated. Pupils equal, reaction not obtained.
Summary of mental: This man shows all evidence of advanced age, hair gray, skin wrinkled, feet swelling when he sits up. He is not oriented in any of the spheres. He is unable to answer any question with intelligence.

Diagnosis: Senile psychosis, simple type.

His death certificate shows cause of death to be broncho pneumonia, which he had for 5 days, with contributing factors being a fractured hip. Shows date of death as May 21, 1926 at Eastern Oklahoma Hospital in Vinita, Oklahoma. Shows length of residence in the hospital to be 1 month and 2 days. Shows that he was buried in the hospital cemetery on May 28, 1926
