Mother was Mary Wakefield, and is dead. Patient is youngest of 5 children. ::: ::: Personal History: Patient says he is 82 years old, and a Veteran of the Indian War. He was born in Michigan and lived there until 18 years of age. He completed the 4th grade and did well in school; quit school because his mother and father were dead and no one sent him. ::: ::: He married Clara Ward and they had 4 children. They were separated and could not agree for 16 years. Then they went back together and remained together until he went to the Soldiers Home in St. James, about 3 1/2 years ago. He has been a coal miner and laborer all his life. ::: ::: Present Illness: He says he has eczema all over his face (none visible) and is generally run down and very weak. He was sent here for medical attention. ::: ::: Habits and Character: He rarely ever drank, never smoked or used tobacco. He is a member of the Baptist Church. Denies venereal disease. He was arrested for petty larceny 20 years ago, but was innocent and turned loose. ::: ::: Stream of Talk: Free, relevant and coherent. He answers all questions willingly and readily. ::: ::: Emotional Status: He says he is not happy away from his wife, but is content to stay here until he can leave. Governor is unknown, but Roosevelt is President. He names 2 of the assassinated Presidents. ::: ::: He married Clara Ward and they had 4 children. They were separated and could not agree for 16 years. Then they went back together and remained together until he went to the Soldiers Home in St. James, about 3 1/2 years ago. He has been a coal miner and laborer all his life.

::: ::: Personal History: Patient says he is 82 years old, and a Veteran of the Indian War. He was born in Michigan and lived there until 18 years of age. He completed the 4th grade and did well in school; quit school because his mother and father were dead and no one sent him.
::: ::: He married Clara Ward and they had 4 children. They were separated and could not agree for 16 years. Then they went back together and remained together until he went to the Soldiers Home in St. James, about 3 1/2 years ago. He has been a coal miner and laborer all his life.
::: ::: Present Illness: He says he has eczema all over his face (none visible) and is generally run down and very weak. He was sent here for medical attention.
::: ::: Habits and Character: He rarely ever drank, never smoked or used tobacco. He is a member of the Baptist Church. Denies venereal disease. He was arrested for petty larceny 20 years ago, but was innocent and turned loose.
