a. FindAGrave memorial, indicates that his father is Warren Church Johnson.
b. As early as his World War I draft papers on 6 September 1918 he is using surname Johnson. Also on the papers he indicates his nearest relative is Mrs. Stella Johnson of Waukon. Until 20 April 2017, there is no indication his mother was married to a Johnson outside of this draft paper.
c. Confusing this is that in 1920 Census his surname is Graham. All other records after the 1918 record has his surname as Johnson.
d. The 1918 prompted more research on Harold's mother which is documented under her name.
