Individual Details
Jack Cogen FLATT
(29 Dec 1928 - 10 Jun 1981)
The Flatt family published knowledge is that Jack was the son of Frank Collins and was raised by his aunt/uncle Elizabeth Collins Flatt and Wesley Flatt and that he took the surname Flatt. This is generally supported by public documents. The 1930 Census lists Jack as the nephew of Wesley and is living with Frank Collins in the Wesley Flatt household. In the 1940 Census he is listed in the Wesley Flatt household as a surnamed Flatt son and is attending fifth grade. To date, no record of a formal adoption is known. Jack's Social Security, Veterans, and death records (lists his mother's maiden name as Collins) all list his surname as Flatt.
He is listed in the Veterans Administration BIRLS Death File 1850 - 2010; he enlisted in the U. S. Army on 6 February 1951 and was released on 25 November 1952. Important to note that the VA has one record with surname Collins and another with surname Flatt.
In December 1960 he was a passenger on Military Air Transport Service Flight 432 from Torrejon Air Base near Madrid Spain which arrived at McGuire Air Base in New Jersey. He was processed by the Immigration and Naturalization Service. There is a possibility he was in the service at this time but no record of it.
It is possible that Jack is the Jack C. who married Sandra E. G. Barnes in Mobile, Alabama in May 1964 but not enough information is provided to make a confirmation.
Jack's last Social Security payment went to an address in Northville, Michigan but his place of death is in Sacramento, California. His social security number was issued in North Dakota prior to 1951.
